For health professionals
Referrals, without the paperwork chase.
We accept referrals from hospitals, GPs, allied health and discharge planners across the Wheatbelt. Urgent post-hospital referrals are triaged the same day.
- Email admin@ruralagedcare.org.au with the client's name, contact details and reason for referral
- Or ring 0418 862 850 and ask for the clinical intake team
- We acknowledge every referral within one business day and confirm the plan in writing

We take clients anywhere in Western Australia — towns, farms and remote properties. Travel outside our local district is planned into the roster and confirmed in writing.
